Understanding the science behind ice cream headaches

That sharp, sudden pain many experience after eating cold treats too quickly-commonly known as brain freeze-may reveal more about personal health than previously thought. Neurologists suggest these brief but intense headaches could be hereditary and may signal underlying migraine conditions.

Why brain freeze happens

Amaal Starling, a neurologist at the Mayo Clinic in Minnesota, explains that cold-stimulus headaches occur when rapid cooling in the mouth or throat causes blood vessels to constrict and then rapidly dilate. Pain receptors in these vessels connect to the trigeminal nerve, which processes facial and forehead pain signals, creating the sensation of pressure in the head rather than the mouth.

While generally harmless, research indicates that cold foods or drinks can occasionally trigger heart palpitations or arrhythmias, particularly in middle-aged men.

Prevention and relief strategies

To avoid brain freeze, Starling advises pacing consumption of cold foods and drinks, allowing the mouth to warm between bites or sips. If pain strikes, pressing the tongue against the roof of the mouth or sipping warm liquid can help shorten the episode.

Family ties and migraine connections

Studies spanning four decades, including research on thousands of children in Taiwan, Germany, and Canada, as well as adult migraine sufferers in Brazil, Turkey, and the UK, reveal patterns in brain freeze susceptibility. Irene Toldo, a child neurology professor at the University of Padua, found that these headaches often run in families, though specific genetic links remain unidentified.

Crucially, individuals who experience migraines report more intense brain freeze episodes. A 1970s study found that 93% of migraine sufferers had experienced ice cream headaches, compared to just one-third of non-migraine sufferers. Starling, who experiences both conditions, notes that heightened trigeminal nerve sensitivity in migraine patients may explain this correlation.

Brain freeze as a research tool

Since the 1960s, scientists have used induced brain freeze to study migraine mechanisms, as both conditions involve the trigeminal nerve. This method provided early insights into blood flow and nerve function related to head pain. While modern research now employs other triggers, such as nitroglycerin infusions, brain freeze remains a valuable model for understanding migraine pathology.

When to seek medical advice

Starling emphasizes that frequent or severe brain freeze could warrant a closer look at overall headache patterns. Migraines affect one in six women, one in eleven children, and one in ten men, yet over half of sufferers never discuss symptoms with a doctor. She encourages those experiencing intense brain freeze to consider whether their non-brain-freeze headaches might indicate a treatable condition.

Enjoying cold treats safely

Despite the risks, Toldo reassures that migraine sufferers need not avoid ice cream entirely. The key, she says, is moderation-savoring cold treats slowly to prevent discomfort while still enjoying the flavors.
