Veterans report breakthroughs after ibogaine therapy

Former US Navy medic Elias Kfoury describes a transformative experience with ibogaine, a powerful hallucinogen being studied for PTSD and traumatic brain injuries. The drug, derived from an African shrub, remains illegal in many countries, including the US, due to safety risks.

From despair to hope: One veteran's journey

Kfoury, who served 21 years in the military, struggled with chronic pain and PTSD after his retirement in 2016. Despite 12 surgeries and multiple therapies, his condition worsened. "I just declined further and further into darkness," he recalls. Born in Beirut during Lebanon's civil war, Kfoury likened his childhood and military service to a continuous state of conflict.

In 2024, Kfoury joined a Stanford University-monitored trial in Tijuana, Mexico, where ibogaine is unregulated. Alongside 29 other special forces veterans, he received a single dose of the drug-up to 14 milligrams per kilogram of body weight-under medical supervision. The experience, lasting up to 72 hours, plunged him into a vivid replay of past memories, including a cathartic conversation with his younger self.

Scientific findings: Improvements but unanswered questions

Post-treatment assessments revealed significant reductions in PTSD, depression, and anxiety among participants. Scores shifted from "mild-to-moderate disability" to "no-to-mild disability." A follow-up study linked the intensity of the drug's hallucinogenic effects to greater symptom improvement, both immediately and one month later.

However, researchers remain uncertain about how ibogaine works. Unlike other psychedelics, it does not primarily interact with the 5-HT2A receptor. Instead, studies suggest it may influence kappa-opioid receptors, which play a role in restoring myelin-a protective coating around brain nerve fibers. Another theory involves noribogaine, a metabolite that increases serotonin activity, potentially stabilizing mood.

Debating the role of hallucinations

Some scientists argue that ibogaine's therapeutic benefits stem from its chemical effects alone. A synthetic, non-hallucinogenic version tested on rats reduced alcohol and heroin-seeking behavior and produced antidepressant-like effects. Startups are now developing similar compounds to replicate these benefits without psychedelic experiences.

Others, like clinical psychologist José Carlos Bouso, believe the drug's vivid, immersive "life review" is central to its healing power. "It's like opening a window of opportunity in which people can make behavioral changes," he says. A 2017 survey of 88 opioid users treated with ibogaine in Mexico found that 67% reported gaining insights into the root causes of their addiction.

Safety risks and regulatory hurdles

Ibogaine carries serious risks, including cardiac arrest. In the Stanford trial, participants received magnesium sulfate to protect their heart rhythms, and medical staff monitored them continuously. Despite these precautions, fatalities have occurred, though data is incomplete due to underreporting by clinics.

Alan Davis, a researcher at Ohio State University, notes that ibogaine's high cost and safety concerns may delay its approval as a mainstream treatment. "There's no uniform trajectory that unfolds after treatment," he says. In his 2017 study, 17% of opioid users reported no change, while 6% said their opioid use increased post-treatment.

Growing momentum despite challenges

Interest in ibogaine is accelerating. In April 2026, US President Donald Trump signed an executive order directing the FDA to expedite psychedelic research, including $50 million for ibogaine studies. Texas had already allocated $50 million in 2025 for clinical trials targeting PTSD, traumatic brain injury, and opioid use disorder.

Many veterans, including Kfoury, credit ibogaine with life-changing results. "Physically, intellectually, emotionally, spiritually-every part of me was touched by that medicine," he says. While his headaches have subsided, he emphasizes that the drug is not a cure-all. "It gave me a roadmap, but you have to put the work in consistently."

What's next?

Researchers stress the need for large-scale, randomized clinical trials to validate ibogaine's efficacy and safety. Complicating matters, some clinics combine ibogaine with 5-MeO-DMT, a potent psychedelic derived from toad venom. While patients describe the pairing as complementary, Davis warns that the lack of research makes it difficult to determine which substance drives the benefits.
